How to Complete the OMR Application Form

Please read the guidelines carefully before filling the OMR Application Form.

General Guidelines:

  • The Application Form will be scanned by Optical Mark Reader (OMR) which is a computer-based machine. Therefore, particular attention should be paid while filling in data in the Application Form.

  • Mark your response only in the space provided for the purpose. Response marked elsewhere will not be considered. Fill the boxes in the form first and then proceed to darken relevant circles.

  • Use HB Pencil only for filling up information items 1 to 12. Please darken the appropriate circles with due care. Fill up item no.13 onwards using black / blue ballpoint pen only. Avoid over-writing.

  • Completely darken the respective circle for your response. You may change a mark by completely erasing the wrongly filled circle. The erasure should be complete and without any smudges as partially erased mark may lead to wrong reading by the computer.

  • Please do not write or mark on this Application Form outside the demarcated areas.

  • Do not pin or staple any other material with the OMR Application Form.

  • The OMR Application Form is supplied in a pre-addressed envelope and the applicants are required to send the duly filled Application Forms in the same envelope to the Admissions Officer,

  • The OMR Application Form should not be folded.

  • Do not make any stray marks on the OMR Application Form.

  • No certificates should be enclosed.

  • The applicants are advised to keep a photocopy of the OMR Application Form.

  • Ensure complete correctness of the data.

  • While stating the year under appropriate items, (eg: date of birth or year of completing an academic qualification), use the standard notation as given in the following examples:

    • Year 1998 should be indicated as

Guidelines for filling individual items

The OMR Application Form contains 17 individual items. All items have to be filled in carefully before dispatching the completed Application Form.

Guidelines for filling the individual items are given below:

1.  Name of the Applicant:

Write your name, in capital letters, in the boxes provided. Leave one box vacant between each initial.

Example:

2.  Application Number:

The OMR Application Form  has a unique 5 digit application number. Write your application number in the boxes and darken the corresponding circles.

3.  Test Center Code:

You can choose any one of the test centers available for the Admission Test as indicated in the next page. In the event of not having enough seats at the test center requested by you, you will be allotted the nearest alternative test center, under intimation to you through E-mail. Campus Programs Admissions Department® (CPAD®) may add few more test centers apart from the test centers given in this application material.

Codes for test centers are given below:

Please note the following code numbers of the test center given in the brackets. Enter the code in the box and darken the corresponding circles under item No.3. As this is very important, please double check the code.

7.  Academic Record (Do not enclose any certificates):

7a)  Class X: 

Aggregate Marks (%): Indicate the percentage of aggregate marks up to first decimal in the boxes provided and darken the appropriate circles.

Year of Passing: Indicate the year in which you have passed Class X.

7b)  Class XI: 

Aggregate Marks (%): Indicate the percentage of aggregate marks up to first decimal in Class XI, in the boxes provided and darken the appropriate circles.

Year of Passing: Indicate the year in which you have passed Class XI.

Note: Candidates studying Class XII should leave item 8(c) blank.

8.  Campus/Branch Preferences:

Please select the desired IcfaiTech Campus/Branch and indicate your preferences in the boxes provided and darken the bubbles corresponding to the appropriate code you have selected. 

  • Candidates applying for ATIT 2008 have to indicate all twelve preferences.

  • Candidates applying on the basis of AIEEE 2008 will be offered admission in IcfaiTech, Dehradun and have to indicate at least four branch preferences at IcfaiTech, Dehradun. However they can also seek admission into IcfaiTech campuses other than Dehradun and have to fillup all twelve preferences.
